Attaching generated PlantUML PNGs to page "accidentally"?

This is a strange one. Very occasionally, I will be notified that someone has added a PNG file that was generated by the PlantUML macro to a page that contains the same UML diagram. They haven't made any other edits on the page or to the diagram or to any other attachments, nor does the new PNG appear anywhere on the page. 

It's almost like an accidental mouse-over and drag-and-drop copied the generated PNG to desktop, then they drag-and-dropped it into the attachments list. But, I doubt that they would have done it on purpose; they probably don't even know how. We're asking one of the users what happened now, but if it was accidental they may not even realize what happened.

Has anyone else seen this happen and do you have any idea what might be occurring? I'm sure it's user error but can't envision what steps would cause this to happen.

This is a followup to my question. I believe this was caused by specifying the output image file name in the macro settings. If this is populated, it seems a refreshed version of the image is generated when someone views the page (I think) which prompts an email notification. 

To test this I went to the pages in question and removed the output image file name in the macro settings. I did not receive any more notifications about new versions of the image being uploaded. 

The mystery appears to be solved as far as our particular instance is concerned!

As I understand, you have an inconsistent issue with file upload. We could move forward by fist checking a few points with your users.

Here is a small list of questions to try and address this:

1. Is this affecting all users or only a small/well-defined group?
2. When did you first notice this issue?
3. Is there any other kind of file that is also affected by this behaviour?
4. What browser were your users using when the issue arose?

With the questions above, we are aiming at slashing some root causes out of the equation, such as an issue that shows up specifically with the plugin you mentioned.

As you have mentioned, getting in touch with the affected users can help to shed some light on how and why this issue happens.

As a note, you might want to look into a newer version of Confluence. Confluence 6.2.3 is not supported for some time now. According to the documentation, the oldest version that is still supported (at the time of writing this post) is Confluence 6.5.x. However, support for it will be dropped on November 1st, 2019. Hi Diego, we asked the two users who'd done this what happened and both of them said they had double-clicked on the image generated by the PlantUML macro. I don't know how that would have attached it to the page again, and I was not able to reproduce. So it remains a mystery!
