Button to open an email

I am using a button with HTML "Mailto" to open a email when clicked. I am able to pre-populate the To, CC, subject and body fields. 

However, i would like to know how i can also add a Form kind of layout to the body. I am trying to setup a form on the body that has fiends like: Name, URL, Feedback type..etc..

I dont want to use the JIRA issue collector at the moment as we are still in the starting stages. 

Please let me know how i can embed a form type layout in the email body.

This requires a 3rd-party add-on since forms aren't native to Confluence.
