Create a form in confluence

Hi There,

I'm trying to create a dropdown list using form for confluence as per the tips that I found on this website https://www.adaptavist.com/doco/display/F4C/Dropdown+List. 

I created a blank page, and did the followings:

Error

Form Configuration "example" not found.

Does anyone know the cause of this problem?

Anyhelp will be appreciated.
