Document generator based on a form

Hi!

In my scenario, the user fills in a web form in Confluence. Based on the values that the user selected, a page is created (based on a template), with said values inserted at specific places in the page.

Ex: User pick which ingredients his receipe contains in a web form, and when he clicks on submit, another page is created containing the name of selected ingredients, along with the way to prepare each one of them.

Do you an app(s) that could achieve this?
