How do I create Timestamp to record time of trigger and user who triggered it?

I need help creating a behavior or listener with Scriptrunner.

User selects "Yes" on "Custom Field 1"

Time is automatically recorded in "Custom Field 2" a Date and Time Picker type

User made the selection/triggered the automation is recorded on in Text field of if Possible User picker" Custom Field 3"

I need assistance in creating something that will perform this. Other add-ons is not an option for us.

As for writing out a full blown script I can't work on this at this specific moment but I hope it would help anyone who could assist you here.
