Plug-in for Signing Policies and Exporting Data?

I am implementing an Acceptable Use Policy and need to collect digital signatures. The plug-in needs to have the ability to capture signatues and export them to excel/word or some other medium for audit purposes. I have looked at Confiforms but it is not an option because of its country of origin. I am in the US and require a plug-in from the US, Australia, Canada, New Zealand, or the United Kingdom. If anyone knows a great plug-in that would fit these needs, please let me know. Thanks!

Both apps provide e-signature capability and you can export the page activity to PDF. You can find them on the Atlassian Marketplace.
