Adding a submission Page

I have to add a submission page where one can add title/description. i was wondering whether it is possible through some forms or there is a plugin. Please suggest something

Not too sure how details your requirement is, however you could do some form operation using the template with variable. If you would like to be able to edit it, you might need to use Scaffolding plugin, which is a commercial plugin.

thnx rian....actuly i want a submission page which has 3 radioa buttons and a text fieldand a diff. text field(content) will open on each radio button, and one more text area for the user...also....thnx for reply agnnn :)

Perhaps Scaffolding plugin would help. However please bear in mind that it is a commercialized plugin.
