I changed the base path of confluence from / to /confluence and now all attachment download links still point to /download instead of /confluence/download. Is the download root absolute and not relative to the confluence base path? Is it separately configurable?

Did you followed the instructions in How to Relocate the Confluence Home Directory? Did you updated the new path in the file <confluence-install-directory>/confluence/WEB-INF/classes/confluence-init.properties?
I did not relocate the confluence home directory, hence the setting in confluence-init.properties is still set to the correct location on the file system. I think it's a rendering problem, because the generated attachment links still refer to http://hostname/download/attachments/xxx/yyyyy.zz instead of http://hostname/confluence/download/attachments/xxx/yyyyy.zz although I changed the base directory to /confluence.
