How to create JIRA issues from Confluence when you have required custom fields?

I'm trying to create a new JIRA issue from Confluence 5.6.6 using the Insert JIRA Issue/Filter macro.  The Create New Issue dialog allows me to select my JIRA instance, project and issue type correctly.  I can see all the fields required (some of which are custom fields).  When I try to submit the form, I get the following error for each custom field:

Field 'customfield_14071' cannot be set. It is not on the appropriate screen, or unknown.

Any ideas?

Confluence only allows the summary field(globally).

Same issue here. I have a custom field that is not an option in the bulk creation. So, it will error out. I can not find where to make the custom field show up in the create step in Confluence.
