How to create "was this article useful?" survey

Hi!

I have this knowledge base http://bmx.maximasist.com.br with Confluence.

I need to measure if the articles are beign useful / helpful to my costumers.

Like this:

I've tried lots of plugins and macros, but none has the result we needed.

Lastly i tried the "Forms for Confluence" plugin, it is almost there but i'm stuck in 2 points:

- In the reports i can't get the source submiting link.

- I still have to click on the submit button to send the survey, i wanned just to click on Yes or No and that's it, no submit button.

So i'm stuck, do you guys have any idea?

If you are using Confluence Cloud you could also look at Page Feedback for Confluence

Alternatively, make the comments "This page was helpful" and "This page was not helpful" and ask the users to "like" one of them. Not anonymous.
