There is a 'Confluence Users Summary' macro which claims to do this, but the last login times are all blank.

Yes. I'd suggest using a marketplace app as I assume you want to bulk deactivate users or action on them somehow. Look at https://marketplace.atlassian.com/search?hosting=dataCenter&product=confluence&query=user%20management to explore some of the apps that will help you with that.
