Generating a traceability report from requirements to test cases

Is anyone using a tool to produce  end-to-end traceability from requirements to test cases and perhaps defects (if linked to TC or Req)? If so, what are you using? Which 3rd party plugin(s)? Customized development?

You need to give more details:

1. How do you represent requirements? JIRA issues?
2. And how do you represent test cases? Executions with Zephyr?
3. And how do you represent the links between them? With issue links?

What I am looking for is what solutions, if any, are being used to generate full traceability reports? We don't have any plugins implemented yes and before I suggest something I am curious to know how others might have accomplished this. A single plugin? Combo or plugins?
