The global header (on all pages) for your site under "look and feel" in the confluence admin menu.

Oh, ok, well use the ! shortcut to get a prompt to upload it, or "insert image" in the body, then it will come out in the page.

Could you explain exactly where you want this image to appear? Bear in mind Confluence only has a header and a body.

One way would be to create a page besides the landing page of your space (let's say it's spacekey is MYSPC) so that it won't be seen when navigating. Inside this page (let's call it MyHeader) you just insert your picture then in the "Space tools" menu, select the "Look and Feel" tab and then "Header and Footer".
Inside the header box, add:
{include:MYSPC:MyHeader}
Please note the header will not appear in the Overview page due to the UI (I don't the know the reason why they removed this...)
If you want this image to be also in the PDF then you'll have to play with CSS code as the PDF Layout settings work only when exporting a space, fully or partially, but not when exporting a page from the "..." menu then "export to PDF".
