Require a comment when checking off an action item

I have meeting notes with a list of action items that are represented by a checkbox.  Does anyone know of a way to have it prompt to add a comment when something is checked off?  In fact it would be really cool if it could just call into the comment function, where it quotes the action item and asked for a comment.

What version of Confluence do you use? Cloud or self hosted?

We host it ourselves and the confluence version is 6.1.3
