AD Lookip from a form

I have created a new hire request form and I would like a field to do a look up from active directory. For example, when they get to the Manager's Name field and they start typing the managers first name I want it populate the field or give a list of the managers with that name. I have the Jira AD sync connected as well as the Deviniti AD enhancement plugin but I don't see a way to get my form to use this custom field... Any ideas on how I can accomplish this?
