How to create a project and set kind of website for end users to send requests??

Dear Team,

 

I would like to create a project and set a kind of website for end users to raise requests in JIRA IT Service Desk. Below are my requirements.

I need log SLA for each ticket.

Priority and etc.

 

Could you please share a document with steps along with screenshots, Thanks.

Jira Service Desk already is "a website for end users to send requests".  Just use it as intended, and you'll find it does everything you've asked for, and a whole lot more.

Apologize denial, but No, thank you, no. And apart from any freelancers (I am not sure if Community is the best approach to address them), I would discourage anyone to do so. And here we go with rationale behind it:

1. JIRA Service Desk offers built in SD Portal for such purpose, plus SLA module
2. If not JIRA SD - JIRA Core/Software has built-in capability of Issue collectors and can be amended with SLA add-on
3. Atlassian is providing good walkthroughs in JIRA documentation, as well as add-ons providers are providing ones - it is very simple (google) to find them
4. Manufacturing such document for You, without more detailed scope, all business variables etc. is a loss of time (from any expert side, but also from yours, as document might be 50% relevant to your use-case and 50% totally off the shelf)
5. Last, but not least - Solution Design activity (designing configuration based on business requirements) is something taking from 1 to few MANDAYS of work, depending on multiple variables. 

I would suggest either:
A. Doing it on your own supported by documentation
B. Hiring freelancer
C. Hiring Atlassian Partner

If by any chance You are not a potential customer, but "wannabe JIRA admin" - A is your preferred answer. If You are looking for commercial application - B or C would do.
