Our requirement is that when participants are added to an issue they get all updates to that issue.
I can't see how to set this up ? i.e. customise the notifications so that they get notifications or are automatically added as watchers when they are added as participants ?
How do I do this ?

Participants get the same notifications as the reporter of the ticket. See Atlassian documentation for more context.

No - that link doesn't tell me anything / anything new. The point is I've added participants to a ticket and they are NOT getting updates.

Request Participants, like organizations, are specific to Jira Service Desk. There is an additional layer to service desk notifications that you can find inside your Service Desk settings. More on this layer:
Customers (these are both the reporter of the ticket and participants to it) get notified of public changes to a ticket. Those are workflow (status) changes and public comments. They also have the ability to opt out of notifications if they want to.
