When putting in a support ticket, what fields/add-ons do Atlassian use?

In particular, the drop down for "Impact to your business" drop down. I would like to format one of my drop downs similar to that. I feel like this should be a no-brainer, but I am blanking on it.

Atlassian isn't using JSD for their support channel currently. With that said, you can still add a custom field of type single select and replicate the behavior you mention. Just create the field, add in the desired values, expose to the create screen, edit the fields for each request type and add the field. Make it required if you like.

I was actually interested in the formatting of the field as well, how it has a title and brief description. Is that possible as well?
