Unable to reindex after creating proforma form issues.
Looking at the issue property via rest api (api/2/issue/{issueIdOrKey}/properties)

I recommend you to contact Atlassian Support (https://support.atlassian.com) with your support zip attached to your ask for the support team to advise the next steps.
Your other option is to identify where the specific FORM is associated with in your project, and then delete it. Afterward, recreate the FORM again to see if the specific FORM is the root cause because it is not properly configured.
