Add-ons Simply Form

Hi,

I install the plugin "Simply-Form" and I have 2 question about it:

 1. Is it possible to create from a form, a task in Jira (that the fields filled in the form will also enter the new task in the appropriate fields)?

 2. Is there an option to create a dialog or comment on the form?
