Copy multiple custom fields value to description field during issue creation

How can I copy values from several custom fields to the description field?

You will need an addon. There are several that can do this.

Scripting ones like ScriptRunner or Power Scripts will give you the most flexibility.

Bob Swift's Update on Transition would also work for this case.
