Custom Form for plugin report

This question is in reference to Atlassian Developer Documentation: Tutorial - Creating a JIRA report

Is it possible to explain how to create the creation form for the creation report?

If I understand the form for launching the report is generated by JIRA based on the atlassian-plugin.xml.

But if I want a customize form ?

I Have issue with a generated form with DatePicker not well initialized (problem with Dateformat (https://answers.atlassian.com/questions/44366907)

You'll need to write your own classes and templates for a completely new form.
