This is a question for JIRA and Confluence. Can we produce a report which shows the access/permissions of each user in a CSV format?

As for JIRA, you can export the user accounts with group membership, roles per project, etc. from JIRA as explained here.
Up to my knowledge, this is not solved for Confluence.
A workaround you could eventually try is that you set up a dummy JIRA instance. Then you share the user accounts (using LDAP or some federated user directory) between your Confluence and the dummy JIRA. And, voila, you can export the Confluence users from JIRA!
