Missing Custom Fields on Screen

please help. the custom fields are added in a default & view screen with screen scheme of both in combination. 

field configuration is set to have all the fields as required, and made sure they are not hidden but when i create the issue type with the create screen, 2 required fields are missing. also, there is one tab that is totally gone but it should be visible as it is set under View Screen in the screen scheme. 

all of these CFs are not hidden at all, i checked the screen and it pointed to my issue type. also, if the CF is a required field, it should say, a required field is not present on the screen. but it didnt say that, it went through with the creation.

Hi,

You have to add the custom fields under edit screen too. You can find the missing fields using the "Where is my field" option to find the fields. Check the below links,
