3rd party apps helping with eliminating paper approval forms

Which 3rd party App (Jira compatible ) would you recommend to help with incorporating the field from approval form (we do use specific  approval forms in our company) into the JIRA platform. What would be great if we could have the fields in JIRA and simply assign to different managers for approval and eliminate the paper forms.

ProForma would be a great solution for eliminating your paper forms. In fact, this was one of the main driver's behind the creation of ProForma in the first place :)

Its not an App per-say but Jira Service Desk actually has this functionality out of the box, -- we use it in our business units as well as our Tech Support, for example:

We want a new contract process approved we add the approvers (and you can do different approvers in different stages) and then they have to use the portal to go and click approve, this gives us who's approved and rejected right in the Issue, and even a history of Bob Declined the first 5 times, but the 6th revision he approved!

You could even feed approvers through the API into your workflows, if you are doing it outside of Jira.
