How can we connect bugs appeared after a certain sprint or story with that sprint/story?

Sometimes after launching a certain task you might notice you broke something in the process. We would really like to keep these things in track.

So at the end of a sprint we will have a chart or some stats like: bugs solved in the process versus new bugs (bugs created in the process). 

+ we would like to know that bugs x,y,z  were 'launched' because of this story/task. 

How do you suggest we do that?
