How to change describe or comment form when I create a issue?

When I create a issue,there should be write describe in blank .

Now I want to change the form of the describe.

The form of the describe is

when :

what :

where :

why :

who :

I want to fasten the "5W" in the describe , then users only write comment after "5w", it just look like this(users only write the bold character)

when: 2012/10/20

what : the jira issue

where: China

why: Jira can't use

who: wangyu

Is there any plugin can solve this problem?

Or Could I make the "5W" to a item ?

You can create a "Unlimited Text Field" custom field and set the default value to your 5 W's.

Then in your field configuration, you the custom field and hide the description field

How could I create a "Unlimited Text Field" custom field ? Do I need install any plugin to create a "Unlimited Text Field" ?

Now I find "Unlimited Text Field" custom filed when I create a field ,But how could I set the default value to 5W . When I select Unlimited Text Field, there only write describe with this field ,could not set the default value to 5W. How could I solve this problem?
