Downloaded & installed Crucible, configured Crucible to talk to JIRA and the external SVN repository.
Created Application and Project links from within JIRA to FishEye/Crucible.
Installed JIRA FishEye plugin and restarted JIRA
On FishEye configuration page in JIRA, set the 'Crucible Integration' to 'yes' which was 'standalone'.
I am still not seeing Review or Source tabs within JIRA. Am I missing any step?
Yes the view permission is already set.

The "Accept remote API calls ON" needs to be turned on both on JIRA and on FishEye/Crucible server side I had it enabled only on the JIRA side.
