Customize Log work Screen

I want to add a Custom Filed in Log Work screen, can we do that? Or do we need to purchase the Add-in for that.

I want to have a checkbox on top of the log work screen, which toggles the usage of some Java code i wrote.

I'm searching for a little tutorial since days...
