Have generic issues added to my new projects

Hi I would like to have a number of generic issues (which would be decision type issues) added to my new project back log, with a Kanban board.  The reason for this is, to have a checklist of questions that force my PMO to capture the right information before progressing to the next stage of engagement of the project.  We also want to capture the results of these questions in reporting.

What is the easiest way to have about 6-8 generic issues added to the backlog of all my new projects?
