I am creating a plugin for JIRA. It works well in my local instance. But, I am getting the below error whenever I use the same in DEV/QA.

The page you were looking for doesn't exist.

You may have mistyped the address or the page may have moved.

Can any1 assist me here to resolve the issue?

The below error was taken from tomcat.log file:

contains form parameters in the request body but the request body has been consumed by the servlet or a servlet filter accessing the request parameters. Only resource methods using @FormParam will work as expected. Resource methods consuming the request body by other means will not work as expected.

Could you tell us more what do you do when the error appears? Do you just access any web-item? What is the code of the action class for this web-item?

Am accessing a web item which will call a servlet then the servlet will fetch the values and it will redirect to a Velocity template page which then display the values. It works in my local JIRA instance. But if I upload the jar and access it from there then it's not working . It says the web page you were looking for does not exist

It's not super well advertised, but there is a specific community site for developers at https://community.developer.atlassian.com/ - you'll more likely get a satisfactory answer there. Sorry for the extra work of posting twice!
