Which stories are shown as completing outside the sprint

I am looking for exhaustive set of criteria which are used to report a story as completed outside the sprint.

1. If Story completed before sprint start even if it was assigned to it.

2. If Story started before sprint start but it completed during this sprint. ( This means it moved to In progress state in say previous sprint and then was moved to new sprint )

3. If Story has future sprint assigned to it but it completes during this sprint.

4. If Story completed after sprint end even if it was assigned to it. ( not sure about this one )

 

Is this correct ? Are there other scenarios.

I see couple of stories which had last sprint assigned and they do not meet any of the criteria above but are still appearing as completed outside the sprint.

3. If the story is NOT assigned to the current sprint, then it will report Closed outside of sprint when the future sprint complete AND the story is/ stays assigned to future sprint

4. The assigned sprint would have to close BEFORE the story resolves (complete or done)

5. If a story has multiple sprints assigned, WHEN the assigned sprint is closed AND the story completes within the assigned sprint, THEN the story completes in the sprint

5a. If a story has multiple sprints assigned, WHEN the sprint closes AND the story closes afterwards, THEN the story closes outside of the sprint

In one case I see multiple sprints assigned but story started and completed within this sprint still it is reported outside the sprint. What could be possible reasons.

Let me check on others and get back.
