How can I insert a comment or free text box within the survey macro?

What is your intention with a comment or a text box? I guess you want to have some kind of subtext or explanation to the actual question you are trying to get votes on, is it?

In that case you can just add a - (hyphen) in the same line of the question and that will be a subtext or description for the question, e.g.:

How do you rate this question? - 5 will be good and 1 will be bad - 1 - 2 - 3 - 4 - 5

the following - will override the default choices
