"misc custom field" plugin, no plugin custom fields at any view

I've installed jira 6.0 and "misc custom fields" version 1.5.6 and encountered following problem: plugin custom fields are not displayed at "create issue" or "edit issue" views at all, even with examples from plugin documentation.

All view options are fine, built-in (not plugin) custom fields works properly.

What causes it?

Have you tried to perform a reindexing process after you installed the plugin? As you may have known, you need to perform this when you are making changes to the Custom Fields as per the following documentation:
