So nothing to do with a button or text field, you want to write a process that creates a test set. Or rather that's just the very last part of your actual desire.
Have a look at the docs for whatever you are creating this "test set" in, and check to see what the API says about creating them. Code for that, and then think about how you want to trigger it, in whatever application is best for hosting a trigger button.
If that application is Jira, you'll need an app that can provide a web-fragment - this is how buttons and other web-items are added to Jira's UI.
