Adding Radio Buttons to Confluence

Hi there

I would like to add yes and no radio buttons to a Confluence page, which sends a mail to a specific person if Yes is chosen. How can I do this in Confluence?

This is not an answer.We also would like to have radio buttons to be used in a template that is used a check sheet to enable  responses "YES" or "NO". Apparently, Confluence does n't have such basic features( Radio button or a check box)

Sorry, not an answer - we also want radio buttons, but we are not enabling HTML (security)
