Error Message: converting checklist (AC) to issues (story)

I'm getting the following error messages when I'm trying to convert a Checklist (acceptance criteria) to a story (issue).  I do not know where to make the change since the issue that has the acceptance criteria has a Team and description.  Please help me find out to correct the errors.

Really sorry for the late reply, it seems that we have missed the post. What you are experiencing is a limitation of the process. The conversion will fail is mandatory fields ,without default values, are in the issue.

However, we are about to release version 5.2 which has an improved conversion tool that will allow you to open the create dialog and then enter the required value.

If you are using the Checklist for Jira app, I suggest that you raise a support request. It will be much more effective to troubleshoot the issue.
