I understand that this plugin is now supported by innova and for a 50-user license, the cost is $240.00. Is that the cost per year? Or is it a one-time charge?

JIRA Misc Workflow Extensions is sold through the Atlassian MarketPlace, which uses the same pricing model as for the Atlassian applications (such as JIRA):

• the initial cost includes a lifetime license and 1 year of support and updates
• support and updates for the subsequent years cost 50% of the full price each year
