Is this plugin compatible with Confluence 5.7.3?

After upgrading, my forms fail to work.  I get an error on the forms that says "Macro name is not unique"

Hi @Christian Burne,

 

Hope this finds you well. The plugin is compatible with Confluence 5.7.3. From Forms for Confluence v 5.0.1 onwards no two macros in a form can have the same name. Please edit the form and give a unique name against the "Name" parameter in each macro to not see any errors. This has been implemented in order to have correct response for each field.
