SQL exception thrown by the Active Objects library: Database

I just installed proFields but i can't create any new profield.

Every time i try, i see this:

What did i do wrong ?

First of all apologies for not replying back earlier.

Please try the latest version of PROFIELDS, you should now be able to create a new project field without any trouble.

In the future please include your Profields, JIRA, database and browser versions as it will help us identify the issue quicker.
