Hi Prerna,

Pardon the delay in responding. There are several options:

1. The Form Setting tab allows you to create rules to automatically add the form, change the status of an issue when a form is submitted, or prevent a transitions if the a form has not been submitted. See the documentation here. 
2. You can also add forms to requests via JSD's automation.
3. Finally, information about using ProForma with Automation for Jira is here.
