I'm having issues with certain team members that change these items under the radar. It would really help to have time stamps/activity for changes to these things - just as we have with comments, and attachments, and even with the moving cards from list to list...
Any suggestions?

I see where you're coming from, but right now, that feature isn’t available within Trello. We actually have it as an open feature request for the product team to consider! I'll add you as an official "+1" in support of this.
Just a thought, that in case you're having trouble with missing card descriptions/titles, this article might help you recover those:

Another idea would be to use a Power-Up that would track your board members' activity, for example. I'm not an active user of those, but you can browse through available options to see if any Power-Up serves your purpose:

Hello Alina - Can you please add me as a +1 for supporting this. This would be a great feature. We like to update our cards in the description vs the chat module and knowing who and when it was done would be awesome.
