My OnDemand set up is
I cannot seem work out how to reference JIRA issues in confluence.
The documentation is confusing around this it seems to jump between documentation that covers OnDemand and documentation that is not applicable.
It suggests that JIRA issues can be inserted in confluence using the 'Insert JIRA Dialog', I can't find any icon or menu item that lets me insert JIRA issues.

Sorry for the confusion in this - to insert a JIRA issue, just click the "insert" button while editing a page:
It's an option in the "insert" option list.

JIRA Issues were not appearing in the insert list. Although they now are as the last item on the list.
