How do I populate a chart with specific information from a template?

I'm looking to use the ""Create from Template" functionality to populate a document that has certain fields at the top for things like Initiative Name, Owner Squad, Quarter, Dependent Squads, etc.  I then want to show this info as a row in a chart or table on a separate page, and each time a new doc was created from the template a new row would be added to the chart or table.  I have a couple questions on how best to do this:

What do I need to use for the chart or table that shows that info as individual rows?

How do I set those specific fields in the template to show in the chart or table?

Hi @TY 

You should be able to do this by adding a page properties macro to your template and then using the page properties report macro to show this information on another page. 

Here you can read more about confluences page properties macro. Let me know if you get stuck somewhere.
