Image Copy & Paste Does Not Work

I cannot insert an image to my wiki document in any way.

The Images icon does not appear in the toolbar, nor in the Insert menu.

Copy and paste does not work.

Drag and drop does not work.

Typing /File displays the Files and Images menu, but clicking it does nothing.

Markup such as ![Alt text](file:///C:\temp\a.jpg "Optional title") does not work.

Any Suggestions?
