Is it possible to create a dynamic form in confluence without any plugin

What you could try is to use google forms and integrate it to your page. But I don't know if this fulfills your goal of a dynamic form. 

Otherwise, I think without plugin, there is no way you reach your indiviual goal.

I think it is worth trying to draw a table in Confluence whilst its cells would be linked with confluence database.

Step1: Create data base in confluence

Step2: Draw and design a form using table object and link cells with confluence database.
