Is there a document generator functionality within Confluence, or Confluence Apps?

I think my team would like to be able to input a template, and specify a form.  Then, subsequent users can fill out the form, and generate documents with the form input substituted into the document template.

 

Is there such a creature that works in the Confluence ecosystem?

Out of the box this functionality is not available, but you can try 3rd party plugins such as confiforms. Confiforms: https://marketplace.atlassian.com/apps/1211860/confiforms-data-forms-workflows?tab=overview&hosting=cloud
