Can you elaborate a bit on what your question is?

What you are showing would be a new Request Type.

Thank you, @Dirk Ronsmans I greatly appreciate it. I am relatively new to Confluence and this is something that I would like to create.

Well the screenshots you show are not the Confluence product.

You'll actually need Jira Service Management for that which will provide you with a portal where you can create these Request Types
