Collect Submission Data into Table

Our purchasing team would like to create a purchase request form that allows the submitter to add multiple items to the request in a table. 

For example: 

  • Questions 2-7 requires the user to enter information about a line item to purchase.
  • Question 8 asks if they would like to add another item. If Yes, it loops them back to question 2. 

Or

  • Is there a way to include a dynamic table to the form that the user can keep adding rows to to add addtiional items to the purchase request? 

This is the example image they provided for what they would like the form to create.

This is not out of the box configuration. You may need to search the marketplace that may extend this capability.

 

Currently, there is a ticket raise with Atlassian on this topic of having tables in JSM forms in the portal.

Technically you can have a table in a Form and the link you provided is closed with a solution. 

I'm looking for table functionality. 

Can you also see this post? JSM Form looping through for repeat data entry (atlassian.com)

This is similar to what I would like to do with the form but rather than Assets be the destination, the form itself or the ticket is the destination. Allow a user to log multiple submissions via the form or the request type.
