I have duplicate fields showing up on the portal view request type. I am not sure why the same field is appearing twice. Anyone else run into this and know the solution besides just deleting and starting over?
Also, I turned on Major Incident and Post Incident review and now my major incident toggle is gone on the issue view.

Are the fields literally identical? If they are, please remove one and the issue will be resolved immediately. I'm not exactly sure how this bug occurs but do know that data will only be stored once for the field so removing the duplicate won't cause any data loss.
I'm not sure exactly what is happening with the major incident toggle but suspect that you need to add the "Major incident" field to the underlying screen and then you will be able to add it to your request type (it currently auto-adds it but this behaviour will change in the coming weeks). The reason for this is that the Major Incident feature changed recently to work via a special custom field that is not automatically added to every existing project.
